• Just fill up all the required information needed (ensure all information is correct)
• Price:
  • Php 300.00 per set of Medical Records
  • Php 150.00 per Chest X-ray CD
  • For shipping additional Php 100.00 will be charged
• Select payment method (Banking, Online Banking, etc.)
  • For banking email a copy of the deposit slip at info@uroic.com with the subject of PAYMENT FOR MEDICAL RECORD (YOUR NAME)
  • For online banking email a screenshot of confirmation payment at info@uroic.com with the subject of PAYMENT FOR MEDICAL RECORD (YOUR NAME)
• Your request will be processed within 3 working days after we received the proof of payment.
• Your Medical Records will be delivered in

  • Provincial (Luzon only) 5 working days
